Child health report
Within 30 days of your child's start date, please have your child's doctor complete this form.
Infant intake form
If your child is under two years old, please complete this form on your child's first day of attendance. It also must be updated every two months to keep us apprised of your infant's schedule.
Authorization to administer medications form
If your child is taking any medications and you would like an Almost Home staff member to administer it during the day, please fill out this form.
